When using the Fillable PDF form, please use the "fill and sign" icon in the toolbar located at the side of the form. Then use the disc icon in the toolbar at the top of the page to save (If you experience a problem with the fillable PDF forms, please use the Word forms.)

Degree Program Students

If using the Word Degree Audit form, do not save it as a PDF document, leave it in the Word format to send it electronically to your advisor. Once your advisor approves your audit, it will be submitted electronically by him/her to the Registrar's Office for approval.
